WEBSITE AND SOCIAL MEDIA RELEASE FORM, THIRD PARTY PHOTOGRAPHY

 

I, the undersigned, do hereby grant permission to The Fannin County Soccer Association to post/release my and/or my child’s story, photo, name or other item, hereinafter referred to as “Materials,” I submit to and for The The Fannin County Soccer Association Web site, Facebook account, and third party photography companies. I hereby release you, your representative, employees, managers, members, officers, parent companies, subsidiaries, and directors, from all claims and demands arising out of or in connection with any use of said “Materials”, including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights. I acknowledge and agree that no sums whatsoever will be due to me as a result of the use and/or exploitation of the “Materials” or any rights therein.

 

AMATEUR ATHLETIC WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to participate in any way in Fannin County Soccer Association athletic sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself and or child from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Fannin County Soccer Association their officers, officials, agents, volunteers and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM

5. Fannin Soccer does not hold medical insurance for players, coaches, refs or volunteers.

 

Refund Policy

Email must be sent to fannincountysoccerassociation@yahoo.com within 7 days of initial payment with the following:

- Bank statement showing FCSA charge

- Player full name

- DOB

- Name on credit card used to pay

- reason for refund request. 

 

After 7 days of initial payment or first scheduled practice, which ever comes first, there will be no refunds.

All refunds will take 3-4 weeks. 

All online fees and late fees are non-refundable.

$15 administration fee will be applied to all refunds.
